ABSTRACT

Our objective is to describe the differences in the sampling plans of the two editions of the Brazilian National Health Survey (PNS 2013 and 2019) and to evaluate how the changes affected the coefficient of variation (CV) and the design effect (Deff) of some estimated indicators. Variables from different parts of the questionnaire were analyzed to cover proportions with different magnitudes. The prevalence of obesity was included in the analysis since anthropometry measurement in the 2019 survey was performed in a subsample. The value of the point estimate, CV, and the Deff were calculated for each indicator, considering the stratification of the primary sampling units, the weighting of the sampling units, and the clustering effect. The CV and the Deff were lower in the 2019 estimates for most indicators. Concerning the questionnaire indicators of all household members, the Deffs were high and reached values greater than 18 for having a health insurance plan. Regarding the indicators of the individual questionnaire, for the prevalence of obesity, the Deff ranged from 2.7 to 4.2, in 2013, and from 2.7 to 10.2, in 2019. The prevalence of hypertension and diabetes per Federative Unit had a higher CV and lower Deff. Expanding the sample size to meet the diverse health objectives and the high Deff are significant challenges for developing probabilistic household-based national survey. New probabilistic sampling strategies should be considered to reduce costs and clustering effects.


Nosso objetivo é descrever as diferenças nos desenhos amostrais das duas edições da Pesquisa Nacional de Saúde (PNS 2013 e 2019) e avaliar como suas mudanças afetaram o coeficiente de variação (CV) e o efeito do desenho (Deff) de alguns dos indicadores avaliados. Variáveis de diferentes partes do questionário foram analisadas para avaliar proporções com diferentes magnitudes. A prevalência de obesidade foi incluída na análise uma vez que a medição de antropometria na pesquisa de 2019 foi realizada em uma subamostra. Os valores do estimador pontual, CV e Deff foram calculados para cada indicador considerando a estratificação das unidades amostrais primárias, a ponderação das unidades amostrais, e o efeito do agrupamento. Para a maioria dos indicadores, CV e Deff foram menores nas estimativas de 2019. Em relação aos indicadores para todos os membros familiares, Deffs foram elevados e atingiram valores superiores a 18 para a posse de um plano de saúde. Quanto aos indicadores no questionário individual, Deff variou de 2,7 a 4,2 em 2013 e de 2,7 a 10,2 em 2019 para a prevalência de obesidade. A prevalência de hipertensão arterial e diabetes por Unidade Federativa apresentou CV maior e Deff menor. A expansão do tamanho da amostra para atender aos diversos objetivos de saúde e Deff altos são desafios expressivos para o desenvolvimento de uma pesquisa nacional domiciliar probabilística. Novas estratégias de amostragem probabilística devem ser consideradas para reduzir custos e efeitos do agrupamento.


Nuestro objetivo es describir las diferencias en los diseños muestrales de las dos ediciones de la Encuesta Nacional de Salud (PNS 2013 y 2019) y evaluar cómo sus cambios afectaron el coeficiente de variación (CV) y el efecto de diseño (Deff) de algunos de los indicadores evaluados. Se analizaron variables de diferentes partes del cuestionario para evaluar proporciones con diferentes magnitudes. La prevalencia de obesidad se incluyó en el análisis, ya que la medición de la antropometría en la encuesta de 2019 se realizó en una submuestra. Los valores del estimador puntual, CV y Deff se calcularon para cada indicador considerando la estratificación de las unidades de muestreo primarias, la ponderación de las unidades de muestreo y el efecto de agrupamiento. Para la mayoría de los indicadores, CV y Deff fueron más bajos en las estimaciones de 2019. En cuanto a los indicadores para todos los miembros de la familia, los Deff fueron altos y alcanzaron valores superiores a 18 por tener un plan de salud. En cuanto a los indicadores del cuestionario individual, Deff osciló entre 2,7 y 4,2 en 2013, y entre 2,7 y 10,2 en 2019 para la prevalencia de obesidad. La prevalencia de hipertensión arterial y diabetes por Unidad Federativa tuvo mayor CV y menor Deff. Un mayor tamaño de la muestra para cumplir con los diversos objetivos de salud y un alto valor de Deff son desafíos importantes para el desarrollo de una encuesta nacional domiciliar probabilística. Se deben considerar nuevas estrategias de muestreo probabilístico para reducir los costos y efectos de agrupamiento.

ABSTRACT

The article describes methodological aspects in defining the study population, sampling plan, and sample weigthing and calibration of effective sample of the Brazilian National Survey on Child Nutrition (ENANI-2019). This population-based household survey assessed breastfeeding and dietary intake, anthropometric assessment of nutritional status, and micronutrient deficiencies by blood biomarkers in children under five years of age. The data were obtained with a probability sample, with stratification by the five geographic regions in the country and clustering by census enumeration areas (CEAs). The sample was calculated at 15,000 households distributed in 1,500 CEAs, with 300 allocated in each of Brazil's five major geographic regions and 10 eligible households per CEA, sampled using inverse sampling. The required population parameters were thus estimated to reach the study's objectives. The basic sampling design weights were calculated as the inverse probabilities of the households' inclusion in the study. Imputation was used to compensate for non-response to items in the target variables, except for data on the blood biomarkers. Finally, calibration used population totals of children in 60 post-strata, defined by cross-classification of the following variables: major geographic region, sex, and age. The final sample included 14,558 children residing in 12,524 households, distributed in 1,382 CEAs in the 26 states of Brazil and the Federal District. The data from the ENANI-2019 survey will support strategies for the promotion and implementation of public policies for children under five years of age.


O objetivo deste artigo é descrever aspectos metodológicos referentes à definição da população da pesquisa, plano amostral, ponderação e calibração da amostra efetiva do Estudo Nacional de Alimentação e Nutrição Infantil (ENANI-2019). Trata-se de um inquérito populacional de base domiciliar que realizou avaliação do aleitamento materno e de consumo alimentar, avaliação antropométrica do estado nutricional, e avaliação das deficiências de micronutrientes mediante análise de biomarcadores sanguíneos em crianças menores de 5 anos de idade. Seus dados foram obtidos por meio de uma amostra probabilística domiciliar, com estratificação geográfica por macrorregião e conglomeração por setores censitários. A amostra foi dimensionada em 15.000 domicílios, distribuídos em 1.500 setores censitários, sendo 300 em cada macrorregião e 10 domicílios elegíveis por setor, através de amostragem inversa. Assim, estimaram-se os parâmetros populacionais requeridos para atingir os objetivos do estudo. Os pesos amostrais básicos do desenho foram calculados como inversos das probabilidades de inclusão dos domicílios na pesquisa. Para compensar a não resposta de itens das variáveis pesquisadas foi usada imputação, com exceção para os dados de biomarcadores sanguíneos. A calibração empregou totais populacionais de crianças para 60 pós-estratos definidos por cruzamento das variáveis macrorregião, sexo e idade. A amostra final compreendeu 14.558 crianças, residentes em 12.524 domicílios, distribuídos em 1.382 setores censitários nas 27 Unidades da Federação. Os dados do ENANI-2019 poderão subsidiar estratégias de promoção e implementação de políticas públicas para crianças menores de 5 anos.


El objetivo de este artículo es describir aspectos metodológicos referentes a definición de la población de la investigación, plan de muestreo, ponderación de la muestra y muestra efectiva del Estudio Nacional de Alimentación y Nutrición Infantil (ENANI-2019). Se trata de una encuesta poblacional de base domiciliaria, que realizó una evaluación de la lactancia materna y de consumo alimentario, así como una evaluación antropométrica del estado nutricional y de las deficiencias de micronutrientes, mediante análisis de biomarcadores sanguíneos en niños menores de cinco años de edad. Sus datos se obtuvieron mediante una muestra probabilística domiciliaria, con estratificación geográfica por macrorregión y conglomerados por sectores censitarios. La muestra se circunscribió a 15.000 domicilios, distribuidos en 1.500 sectores censitarios, encontrándose 300 en cada macrorregión, junto 10 domicilios elegibles por sector, a través de un muestreo inverso. De esta forma, se estimaron los parámetros poblacionales requeridos para alcanzar los objetivos del estudio. Los pesos básicos de las muestras del diseño se calcularon como inversos a las probabilidades de inclusión de los domicilios en la investigación. Para compensar la no respuesta de ítems de las variables investigadas se usó la imputación, con excepción de los datos con biomarcadores sanguíneos. La calibración empleó totales poblacionales de niños para los 60 post estratos, definidos mediante el cruce de las variables macrorregión, sexo y edad. La muestra final comprendió 14.558 niños, residentes en 12.524 domicilios, distribuidos en 1.382 sectores censitarios dentro de las 27 Unidades de la Federación. Los datos del ENANI-2019 podrán apoyar estrategias de promoción e implementación de políticas públicas para niños menores de cinco años.

ABSTRACT

RESUMEN El proceso de formación y superación permanente o continuada de los profesionales, como un principio de la Educación Médica, se articula con la integración docencia, asistencia e investigación. Para responder a las exigencias de la actividad investigativa, la búsqueda y el tratamiento de la información requiere de un estudio estadístico para que aflore la cualidad desde el análisis cuantitativo; para ello, es importante considerar que en todos los estudios de esta magnitud, la población total se hace muy engorrosa, cara y lenta, por lo que se deben emplear técnicas de muestreo para obtener información significativa con el uso de menos recursos y menor tiempo. El objetivo del presente trabajo es reflexionar sobre el uso del muestreo en las ciencias médicas. Se expone la fórmula del muestreo para media y varianza conocidas, se da una técnica para encontrar el número de elementos de la muestra inicial cuando no se tienen conocimientos previos de media y varianza; se estudia el muestreo aleatorio simple de proporciones con las probabilidades de éxito y fracaso p y q respectivamente, explicándose el caso cuando se tiene p = q = 0.5 para la obtención de un tamaño de la muestra máximo.


ABSTRACT The objective of this paper is to reflect some considerations on the use of sampling in health sciences, based on the need for professionals in these sciences to familiarize themselves with scientific research in their area of knowledge, from a scientific perspective, that makes it possible to investigate the health problems that are constantly developing. The process of Permanent and Continuous Training and Improvement of professionals, as a principle of Medical Education, is articulated with that of Teaching - Assistance and research, the search and treatment of information in this area of scientific knowledge, requires of a statistical study that emerges the quality from the quantitative analysis, for this, it is important to consider that in all studies of this magnitude, the total population becomes very cumbersome, expensive and slow, so sampling techniques should be used to Obtain meaningful information with the use of less resources and less time. The sampling formula for known mean and variance is presented, a technique is given to find the number of elements of the initial sample when there is no prior knowledge of mean and variance, simple random sampling of proportions with the probabilities of success is studied and failure p and q respectively, explaining the case when you have p = q = 0.5 to obtain a maximum sample size.

ABSTRACT

The spotted-wing drosophila, Drosophila suzukii Matsumura, is considered the main insect pest of small fruit in the world. Identifying susceptible hosts is essential to develop management strategies. The aim of this study was to verify the level of D. suzukii infestation in blackberry, blueberry, strawberry, cattley guava and Surinam cherry fruits cultivated in the southern region in Brazil, and to determine the infestation index (II) per fruit. The studies were carried out during two harvests (2015/16 and 2016/17) in three areas with organic cultivation of small fruit. The highest level of D. suzukii natural infestation was observed in blackberry (40 to 65% infestation), and strawberry (approximately 30% infestation) fruits. In contrast, blueberries were less preferred (< 7% infestation). For native fruit of the region (cattley guava and Surinam cherry), it was also verified infestation of the fly, demonstrating they are hosts for multiplication in the off-season periods of the other crops. In relation to II, blackberry and Surinam cherry provided a higher average number of insects per gram of fruit (≈ 1.6 adults of D. suzukii). In contrast, blueberries and cattley guava had the lowest insect ratio per gram of fruit (≈ 0.7 adults per gram). The period between late spring to mid-autumn was considered ideal for the pest occurrence in the field. The knowledge of natural infestation levels is essential for understanding the behavior of the pest in the field, which will assist in the design of management strategies.(AU)


A drosófila-da-asa-manchada, Drosophila suzukii Matsumura, é considerada o principal inseto-praga de pequenos frutos no mundo. A identificação de hospedeiros suscetíveis é essencial para o desenvolvimento de estratégias de manejo. O objetivo deste estudo foi verificar o nível de infestação de D. suzukii em frutos de amoreira-preta, mirtileiro, morangueiro, araçazeiro e pitangueira cultivados na região no sul do Brasil e determinar os índices de infestação (II) por fruto. Os estudos foram realizados durante duas safras (2015/16 e 2016/17) em três áreas com cultivo orgânico de pequenas frutas. O maior nível de infestação natural de D. suzukii foi observado em amora-preta (40 a 65% de infestação) e morango (aproximadamente 30% de infestação). Em contraste, o mirtilo foi menos preferido (< 7% de infestação). Para as frutas nativas da região (araçá e pitanga), também ocorreu infestação da mosca, demonstrando serem hospedeiros potenciais para a multiplicação da praga nos períodos de entressafras. Em relação ao II, amoras e pitangas proporcionaram um maior número médio de insetos por grama de fruto (≈ 1,6 adultos de D. suzukii). Por outro lado, mirtilos e araçás apresentaram a menor relação de insetos emergidos por gramas de frutos (≈ 0,7 adultos por grama). O período entre final da primavera até meados do outono foi considerado ideal para a ocorrência da praga no campo. O conhecimento dos níveis de infestação natural é fundamental para o entendimento do comportamento da praga no campo, o que auxiliará no delineamento de estratégias de manejo.(AU)

Objetivo: Descrever as características das mulheres trabalhadoras do sexo (MTS) de 12 capitais brasileiras. Métodos: Estudo transversal de vigilância biológica e comportamental com MTS recrutadas por respondent-driven sampling (RDS), em 2016. Foram estimadas, por cidade, as prevalências de HIV, características sociodemográficas, comportamentais e da profissão. Utilizou-se o teste do Qui-Quadrado para comparar essas distribuições. Resultados: Entre 4.328 MTS, a prevalência da infecção pelo HIV foi de 5,3% (IC95% 4,5;6,3), variando de 0,2% (IC95% 0,0;1,2), em Campo Grande, a 18,2% (IC95%13,2;24,7) em Salvador. Em metade das cidades, a prevalência de HIV nas amostras foi >5,0%, enquanto Campo Grande, Brasília e Belo Horizonte apresentaram prevalências <1,0%. Diferenças significativas entre as cidades foram encontradas nas distribuições segundo escolaridade, renda, local de trabalho, idade de início do trabalho sexual, e uso de drogas ilícitas. Conclusão: As características das amostras em cada cidade foram distintas, e influenciaram a prevalência local de HIV.


Objetivos: Describir las características de las mujeres trabajadoras sexuales (MTS) de 12 ciudades brasileñas. Métodos: Estudio transversal de vigilancia biológica y conductual con MTS reclutadas por Respondent-Driven Sampling (RDS) 2016. La prevalencia del VIH, las características sociodemográficas, de comportamiento y relacionadas con la profesión se estimaron por ciudad. Se utilizó la prueba de Chi cuadrado para comparar esas distribuciones. Resultados: Entre 4,328 MTS, la prevalencia del VIH fue 5,3% (IC95%4,5; 6,3), variando de 0,2% (IC95% 0,0;1,2) en Campo Grande a 18,2% (IC95% 13,2; 24,7) en Salvador. En la mitad de las ciudades, la prevalencia del VIH fue >5,0%, mientras que Campo Grande, Brasíla y Belo Horizonte tuvieron prevalencia de, <1,0%. Se encontraron diferencias significativas entre las ciudades en las distribuciones, de acuerdo con la escolaridad, ingresos, lugar de trabajo, edad de inicio del trabajo sexual y uso de drogas ilícitas. Conclusión: Las características de las muestras locales fueron diferentes e influyeron en la prevalencia local del VIH.


Objective: To describe the characteristics of female sex workers (FSW) in 12 Brazilian cities. Methods: This was a cross-sectional biological and behavioral surveillance study with FSW recruited by respondent-driven sampling (RDS) in 2016. HIV prevalence, sociodemographic, behavioral and sex work profession variables were estimated by city. The chi-square test was used to compare these distributions. Results: Among 4,328 FSW, HIV prevalence was 5.3% (95%CI 4.5;6.3), ranging from 0.2% (95% CI 0.0;1.2), in Campo Grande, to 18.2 (95%CI 13.2;24.7) in Salvador. In half of the cities, HIV prevalence in the samples was >5.0%, while Campo Grande, Brasília and Belo Horizonte had prevalence rates <1.0%. Significant differences between cities were found in distributions according to educational level, income, workplace, age sex work started, and illicit drug use. Conclusion: The characteristics of the samples in each city were distinct and have influenced local HIV prevalence.

ABSTRACT

ABSTRACT OBJECTIVE To describe the sampling strategy of an epidemiological survey with the aid of satellite images, including details of the multistage probability sampling process. METHODS A probability sample of individuals living in the rural area of Rosário do Sul, state of Rio Grande do Sul, Brazil, aged 15 years old or more, was evaluated. Participants answered questionnaires (medical history, sociodemographic characteristics, habits, alcohol use, quality of life, stress, rumination, and self-perceived periodontal diseases), and were subjected to clinical oral examinations as well as anthropometric measurements (blood pressure, height, weight, abdominal and waist circumferences). Oral evaluation comprehended a complete periodontal exam at six sites per tooth, including the following assessments: furcation involvement; dental abrasion; tooth decay, including the indexing of missing and filled surfaces; O'Brien index; gingival abrasion; oral cavity and lip lesions; complete periapical radiographic exam, and use of prostheses. Besides this oral clinical approach, subgingival plaque, crevicular gingival fluid, saliva, and blood samples were collected. Examiners were trained and calibrated during previous evaluations. A pilot study allowed the logistic of the performed exams to be adjusted as needed. RESULTS Among 1,087 eligible individuals, 688 were examined (63.3%). Age, sex, and skin color data were compared to data from the last demographic census (2010) of the Brazilian Institute of Geography and Statistics, which served to validate the sampling strategy. CONCLUSIONS The careful methods used in this study, in which satellite images were used in the delimitation of epidemiological areas, ensure the quality of the estimates obtained and allow for these estimates to be used in oral health surveillance and health policies improvements.

RESULTS There were interviews in 25,000 homes, surveying 79,874 people, of which 62,659 were affiliated to EsSalud. The insured people are mainly males (50.6%) with a higher technical education level (39.7%). The insured population has mostly independent (95.0%) and own (68.1%) home. Only 34.5% of the insured practice some sport or physical exercise; 14.0% of the population suffers from a chronic disease; 3.5% have diabetes; and 7.1%, arterial hypertension. In the last three months, 35.4% of the members needed medical attention; of these, only 73.1% received health care and the remaining 10.9% were treated in pharmacies or non-formal health care services. RESULTS The 25,000 homes were interviewed, surveying 79,874 people, of which 62,659 were affiliated to EsSalud. RESUMEN OBJETIVO Reportar el diseño, metodología y resultados iniciales de la Encuesta Nacional Socioeconómica de Acceso a la Salud de los Asegurados de EsSalud. MÉTODOS La Encuesta se ejecutó en los 24 departamentos del país. Los temas investigados fueron: características de la vivienda y miembros del hogar, educación, salud, empleo e ingreso y gastos del hogar. Se realizó un tipo de muestreo bi-etápico: la unidad primaria de muestreo estuvo conformada por conglomerados compuestos por una manzana dentro del ámbito de cobertura del centro asistencial; la unidad secundaria de muestreo fueron viviendas particulares donde habitaba al menos un asegurado a EsSalud. Se analizaron los datos de 62,659 afiliados y se muestran porcentajes ajustados por el factor de expansión. Para las comparaciones, se utilizó la prueba de chi-cuadrado. RESULTADOS Se entrevistaron 25000 viviendas, encuestándose a 79,874 personas, de las cuales 62,659 eran afiliados a EsSalud. Los afiliados son principalmente varones (50.6%) con un nivel de educación técnico superior (39.7%). La población afiliada cuenta mayoritariamente con vivienda independiente (95.0%) y propia (68.1%). Solo el 34.5% de los asegurados practica algún deporte o ejercicio físico. El 14.0% de la población padece de alguna enfermedad crónica; el 3.5% presenta diabetes; y el 7.1%, hipertensión arterial. En los últimos tres meses, el 35.4% de los afiliados necesitaron atención médica; de estos, solo el 73.1% recibieron atención sanitaria y el 10.9% restante se atendieron en farmacias o servicios no formales de atención en salud. CONCLUSIONES Esta encuesta es la primera realizada en la población de afiliados a EsSalud, aplicada a nivel nacional, y cuenta con datos socioeconómicos y demográficos de los asegurados, su distribución, factores de riesgo de la salud, prevalencia de los problemas de salud y el grado de acceso a los servicios de salud.

ABSTRACT

RESUMO: Introdução: O artigo teve o objetivo de descrever a metodologia de coleta dos dados dos exames laboratoriais da Pesquisa Nacional de Saúde (PNS). Metodologia: Foi selecionada uma subamostra de 25% dos setores censitários, obedecendo à estratificação da amostra da PNS, com probabilidade inversamente proporcional à dificuldade de coleta. A coleta de sangue e urina dos moradores selecionados para entrevista individual foi realizada nos domicílios por um agente de laboratório. Por conta das dificuldades encontradas no trabalho de campo,a amostra não atingiu número suficiente em alguns estratos da pesquisa, então para a análise dos dados foi proposto procedimento de pós-estratificação. Resultados: A coleta de material biológico foi realizada em 8.952 indivíduos. Os exames realizados foram: hemoglobina glicada; colesterol total; colesterol LDL; colesterol HDL; sorologia para dengue; hemograma série vermelha (eritograma) e série branca (leucograma); cromatografia líquida de alta eficiência (HPLC) para diagnóstico de hemoglobinopatias; e creatinina. Na urina, estimativa de excreção de potássio, sal, sódio e creatinina. A base de dados dos exames laboratoriais foi ponderada e disponibilizada para os usuários no site da PNS da Fundação Oswaldo Cruz, sem necessidade de autorização prévia para uso. Conclusão: A subamostra total coletada é de grande valia, pois permitiu estabelecer parâmetros de referência nacionais adequados às características sociodemográficas e geográficas da população brasileira, fornecendo informações relevantes e complementares para a análise da situação de saúde do Brasil.


ABSTRACT: Introduction: This article aims at describing the National Health Survey (Pesquisa Nacional de Saúde- PNS) methodology of collecting laboratory exams data. Methodology: A subsample of 25% of the census tracts was selected, according to the stratification of the PNS sample, with a probability inversely proportional to the difficulty of collection. The collection of blood and urine was done in the households by a laboratory agent, among residents selected for individual interview. Due to the difficulties found in the field work, the sample did not reach the minimum expected number in some strata, and a post-stratification procedure was proposed for the data analysis. Results: The collection of biospecimens was performed in 8,952 individuals. Laboratory tests were: glycated hemoglobin; total cholesterol; LDL cholesterol; HDL cholesterol; serology for dengue; red blood cell count (erythrogram) and white series count (leukogram); high performance liquid chromatography (HPLC) for diagnosis of hemoglobinopathies; creatinine. Theexcretion of potassium, salt and sodium and creatinine was estimated in the urine. The database of laboratory exams was weighed and made publicly available on the Oswaldo Cruz Foundation's PNS website and can be accessed without prior authorization. Conclusion: The total subsample of laboratory exams is of great value, since it allowed us to establish national reference parameters adequate to sociodemographic and geographic characteristics of the Brazilian population, providing relevant and complementary information for the analysis of the health situation of Brazil.

ABSTRACT

The size of the non-sampling error is directly related to the accuracy and reliability of the sampling survey result. This paper studied the non-sampling errors generated during the sampling process of the China National Human Biomonitoring Program(CNBP), mainly including the sampling frame error, non-response error and measurement error. The program reduced the influence of the non-sampling error on the quality of the survey effectively by scientifically designing the sampling scheme and questionnaire, strengthening investigator trainings and standardizing the data review, which could be used to provide reference for the control of non-sampling errors in public health monitoring projects in China.

RESUMO Trata-se de um estudo observacional transversal, objetivando a apresentação de indicadores relacionados às mulheres que fazem uso excessivo de álcool. Devido à dificuldade de obtenção de um cadastro que permitisse a seleção aleatória de mulheres alcoolistas, decidiu-se como alternativa a aplicação do método Respondent Driven Sampling (RDS). O método permitiu a apresentação de indicadores relacionados ao alcoolismo com base em uma amostra de 36 mulheres selecionadas de centros de tratamento. Os resultados da pesquisa evidenciaram que dos 91,7% das mulheres que fazem uso excessivo de álcool em conjunto com outras drogas, 69,44% usam maconha e cigarro. E, das 30.087 mulheres que consomem álcool, 24.371 necessitam de avaliação e tratamento.


ABSTRACT In this cross-sectional observational study, our focus is to show statistics related to women who use excessive alcohol. Howewer, the difficulty to obtain a frame to consider a sampling design of alcoholic women, it was decided as an alternative the use of the Respondent Driven Sampling (RDS) method. The method allowed the presentation of indicators related to alcoholism based on a sample of 36 women selected from treatment centers. The results of the survey showed that of 91.7% of women who make excessive use of alcohol in combination with other drugs, 69.44% use marijuana and cigarettes. And, of the 30,087 women who consume alcohol, 24,371 need evaluation and treatment.

La representatividad de una muestra, permite extrapolar y por ende generalizar los resultados observados en ésta, a la población accesible; y a partir de ésta, a la población blanco. Por ende, una muestra será representativa o no; sólo si fue seleccionada al azar, es decir, que todos los sujetos de la población blanco tuvieron la misma posibilidad de ser seleccionados en esta muestra y por ende ser incluidos en el estudio; y por otro lado, que el número de sujetos seleccionados representen numéricamente a la población que le dio origen respecto de la distribución de la variable en estudio en la población, es decir, la estimación o cálculo del tamaño de la muestra. Es así como el análisis de una muestra permite realizar inferencias, extrapolar o generalizar conclusiones a la población blanco con un alto grado de certeza; de tal modo que una muestra se considera representativa de la población blanco, cuando la distribución y valor de las diversas variables se pueden reproducir con márgenes de error calculables. Entonces, el muestreo tiene por objetivo estudiar las relaciones existentes entre la distribución de una variable en la población blanco y las distribución de ésta variable en la muestra a estudio. Para ello, es fundamental, entre otras cosas definir los criterios de inclusión (características clínicas, demográficas, temporales y geográficas de los sujetos que componen la población en estudio) y de exclusión (características de los sujetos que pueden interferir con la calidad de los datos o la interpretación de los resultados. El objetivo de este manuscrito, es entregar conocimientos generales respecto de las técnicas de muestreo más utilizadas en investigación clínica.


The representativeness of a sample allows extrapolating and therefore generalizes the results observed in this, the accessible population, and from this, to the target population. Thus, a sample will be representative or not, only if it was selected at random, i.e., that all the subjects of the target population had the same possibility of being selected in this sample and therefore be included in the study, and on the other hand, that the number of subjects selected numerically represent the population that gave rise to it with respect to the distribution of the variable under study in the population, that is, the estimation of the sample size. Consequently, the analysis of a sample allows us to make inferences or generalize conclusions to the target population with a high degree of certainty, such that a sample is considered representative of the target population, when the distribution and value of the different variables can be reproduced with calculable error margins. So, the aim of sampling is to study the relationships between the distribution of a variable in the target population and the distribution of the same variable in the study sample. For this purpose, it is essential, among other things, to define the inclusion criteria (clinical, demographic, temporal and geographical characteristics of the subjects that make up the study population) and the exclusion criteria (characteristics of the subjects that may interfere with the quality of the data Or the interpretation of results). The aim of this manuscript is to provide general knowledge regarding sampling techniques most commonly used in clinical research.

ABSTRACT OBJECTIVE To describe the sample plan, operational aspects, and strategies used in the 2009/2010 and 2013/2014 EpiFloripa Aging Study. METHODS The EpiFloripa Aging is a population-based longitudinal study with 1,705 older adults (60 years or more) living in the municipality of Florianópolis, State of Santa Catarina, Brazil, in 2009/2010 (baseline). The research was conducted with a face-to-face interviews, organized into blocks of identification, socioeconomic, mental health, health and life habits, global functionality, falls, physical activity, morbidities, use of health services, use of medications, food, oral health, and violence, evaluated in the first (2009/2010) and in the second wave (2013/2014). Additionally, in the second wave, we investigated the issue of discrimination and quality of life. RESULTS The response rate of the first wave was 89.2% (n = 1,705). The baseline sample, with predominance of women (63.9%), was similar to the 2010 Census regarding age for women and slightly different for younger men. In the second wave, 1,197 participants were interviewed (response rate of 70.3%). Follow-up losses were only observed for the variable age group (p = 0.003), and predominantly for those aged 80 years or more. Mortality data linkage and active search for participants were used as a follow-up strategies. CONCLUSIONS This study used strategies that were able to help locate the participants and maintain adherence, which ensured a good response rate during investigations.


RESUMO OBJETIVO Descrever o plano amostral, aspectos operacionais e estratégias utilizadas no Estudo EpiFloripa Idoso 2009/2010 e 2013/2014. MÉTODOS O EpiFloripa Idoso é um estudo de base populacional com 1.705 idosos (60 anos ou mais) residentes no município de Florianópolis, SC, em 2009/2010 (linha de base). A investigação foi realizada por meio de entrevista face a face, organizada nos blocos de identificação, geral (características socioeconômicas), saúde mental, saúde e hábitos de vida, funcionalidade global, quedas, atividade física, morbidades, uso de serviços de saúde, uso de medicamentos, alimentação, saúde bucal e violência, avaliados na primeira (2009/2010) e na segunda onda (2013/2014). Adicionalmente, na segunda onda, investigou-se a temática de discriminação e qualidade de vida. RESULTADOS A taxa de resposta na primeira onda foi de 89,2% (n = 1.705). A amostra da linha de base, com predomínio de mulheres (63,9%), foi semelhante ao Censo 2010 em relação à idade nas mulheres e ligeiramente diferente nos homens mais jovens. Na segunda onda, 1.197 participantes foram entrevistados (taxa de resposta de 70,3%). Houve perda de seguimento somente para a variável faixa etária (p=0,003), principalmente naqueles com 80 anos ou mais. Utilizou-se como estratégias de seguimento o relacionamento de dados e a busca ativa. CONCLUSÕES O presente estudo utilizou estratégias que conseguiram auxiliar na localização dos participantes e manutenção da aderência, o que garantiu boa taxa de resposta durante as investigações.

ABSTRACT

Abstract: Sampling strategies such as respondent-driven sampling (RDS) and time-location sampling (TLS) offer unique opportunities to access key populations such as men who have sex with men (MSM) and transgender women. Limited work has assessed implementation challenges of these methods. Overcoming implementation challenges can improve research quality and increase uptake of HIV services among key populations. Drawing from studies using RDS in Brazil and TLS in Peru, we summarize challenges encountered in the field and potential strategies to address them. In Brazil, study site selection, cash incentives, and seed selection challenged RDS implementation with MSM. In Peru, expansive geography, safety concerns, and time required for study participation complicated TLS implementation with MSM and transgender women. Formative research, meaningful participation of key populations across stages of research, and transparency in study design are needed to link HIV/AIDS research and practice. Addressing implementation challenges can close gaps in accessing services among those most burdened by the epidemic.


Resumo: Estratégias de amostragem como respondent-driven sampling (RDS) e time-location sampling (TLS) são importantes opções metodológicas para acessar populações chaves para a epidemia de HIV/AIDS como homens que fazem sexo com homens (HSH) e mulheres transexuais. Porém são poucos os estudos que avaliam desafios de implementação desses métodos. Objetivamos apresentar os principais desafios na implementação dos RDS no Brasil e TLS no Peru. No Brasil, o local da pesquisa, a seleção das sementes, e o tipo de ressarcimento foram desafios na implementação do RDS-HSH. E no Peru, as questões geográficas, preocupações com segurança, e o tempo necessário para participar no estudo complicaram o TLS com HSH e mulheres transexuais. Pesquisa formativa, participação das populações chaves em todas as fases da pesquisa, e transparência no desenvolvimento do estudo são necessários para utilização prática dos resultados. Melhorar a implementação dessas pesquisas pode contribuir para aumentar o acesso aos serviços de prevenção e controle do HIV/AIDS entre aqueles que mais precisam.


Resumen: Estrategias de muestra como respondent-driven sampling (RDS) y time-location sampling (TLS) son importantes opciones metodológicas para acceder a poblaciones claves para la epidemia de VIH/SIDA como los hombres que tienen sexo con hombres (HSH) y mujeres transexuales. No obstante, son pocos los estudios que evalúan los desafíos de implementación de esos métodos. Tenemos como objetivo presentar los principales desafíos en la implementación de los RDS en Brasil y TLS en Perú. En Brasil, el lugar de la investigación, la selección de las muestras, y el tipo de compensación fueron desafíos en la implementación del RDS-HSH. Y en Perú, las cuestiones geográficas, preocupaciones con la seguridad, y el tiempo necesario para participar en el estudio complicaron el TLS con HSH y mujeres transexuales. Se trata de una investigación formativa, con participación de las poblaciones claves en todas las fases de la investigación, y transparencia en el desarrollo del estudio, todo ello necesario para la utilización práctica de los resultados. Mejorar la implementación de esas investigaciones puede contribuir a aumentar el acceso a los servicios de prevención y control del VIH/SIDA entre aquellos que más lo necesitan.

ABSTRACT OBJECTIVE To describe methodological aspects of the household survey National Survey on Access, Use and Promotion of Rational Use of Medicines (PNAUM) related to sampling design and implementation, the actual obtained sample, instruments and fieldwork. METHODS A cross-sectional, population-based study with probability sampling in three stages of the population living in households located in Brazilian urban areas. Fieldwork was carried out between September 2013 and February 2014. The data collection instrument included questions related to: information about households, residents and respondents; chronic diseases and medicines used; use of health services; acute diseases and events treated with drugs; use of contraceptives; use of pharmacy services; behaviors that may affect drug use; package inserts and packaging; lifestyle and health insurance. RESULTS In total, 41,433 interviews were carried out in 20,404 households and 576 urban clusters corresponding to 586 census tracts distributed in the five Brazilian regions, according to eight domains defined by age and gender. CONCLUSIONS The results of the survey may be used as a baseline for future studies aiming to assess the impact of government action on drug access and use. For local studies using a compatible method, PNAUM may serve as a reference point to evaluate variations in space and population. With a comprehensive evaluation of drug-related aspects, PNAUM is a major source of data for a variety of analyses to be carried out both at academic and government level.


RESUMO OBJETIVO Descrever aspectos metodológicos do inquérito domiciliar da Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos (PNAUM) quanto ao desenho e implementação da amostragem e da amostra efetivamente obtida, seus instrumentos e implementação do campo. MÉTODOS Estudo transversal de base populacional com amostra probabilística em três estágios da população residente nos domicílios localizados na zona urbana do Brasil. O trabalho de campo foi desenvolvido entre setembro de 2013 e fevereiro de 2014. O instrumento de coleta de dados incluiu questões relativas a: informações do domicílio, dos moradores e dos entrevistados; doenças crônicas e medicamentos utilizados; uso de serviços de saúde; doenças e eventos agudos tratados com medicamentos; uso de contraceptivos; uso de serviços de farmácia; comportamentos que podem afetar o uso de medicamentos; bulas e embalagens; estilo de vida e planos de saúde. RESULTADOS Foram realizadas 41.433 entrevistas em 20.404 domicílios e 576 conglomerados que correspondem a 586 setores censitários distribuídos nas cinco regiões do Brasil, segundo oito domínios definidos por grupos de idade e sexo. CONCLUSÕES Os resultados obtidos no inquérito podem ser utilizados como uma linha de base para futuros estudos que pretendam avaliar o impacto de ações governamentais nas áreas de acesso e de utilização de medicamentos. Para estudos locais que venham a usar um método compatível, a PNAUM pode servir como ponto de referência para avaliação de variações do espaço e da população. Com ampla avaliação dos aspectos relacionados aos medicamentos, a PNAUM é uma grande fonte de dados para as mais variadas análises, que podem ser conduzidas tanto no meio acadêmico como no âmbito governamental.

ABSTRACT

Objective To study the life quality status of left-behind children and its influencing factors by a sampling survey comparing with the non-left-behind children in Chongqing, so as to provide a basis for improving children's quality of life. Methods We adopted a multistage cluster sampling and conducted surveys by home visiting; the children's quality of life was assessed with Chinese version of PedsQL4.0 children's quality of life measuring scale. The life quality status of left-behind and non-left-behind children was evaluated by their scale scores, and the factors influencing children's quality of life were analyzed by multiple-factor analysis. Results The scores of total score of life quality, physiological function, emotional function, social function, and role (performance in school) function of left-behind children in Chongqing were 82.50±11.56, 87.03±13.58, 77.55±16.05, 85.64±14.85, and 77.05±14.22, respectively; and the scores for un-left-behind children were 86.72±10.12, 89.96±11.67, 83.26±14.65, 90.66±12.44, and 81.08±13.67, respectively. The total score and scores of each dimension of left-behind children were all significantly lower than those of non-left-behind children (P<0.01). Multiple linear regression analysis found that the left-behind status, age, and per capita expenditure of a family were the main factors influencing the children's quality of life, with the left-behind status having the greatest influence; moreover, the left-behind status could greatly influence the emotional function, social function and role function. Conclusion To reduce “left-behind” phenomenon is a direct method to improve children's quality of life and increase their physical and psychological health. When “left-behind” phenomenon can not be completely eliminated, families, schools and society should pay more attention to left-behind children, making joint efforts to protect the physical and psychological health of left-behind children.

Multistage sampling is a frequently-used method in random sampling survey in public health.Clustering or independence between observations often exists in the sampling,often called complex sample,generated by multistage sampling.Sampling error may be underestimated and the probability of type Ⅰ error may be increased if the multistage sample design was not taken into considerationin analysis.As variance (error) estimator in complex sample is often complicated,statistical software usually adopt ultimate cluster variance estimate (UCVE) to approximate the estimation,which simply assume that the sample comes from one-stage sampling.However,with increased sampling fraction of primary sampling unit,contribution from subsequent sampling stages is no more trivial,and the ultimate cluster variance estimate may,therefore,lead to invalid variance estimation.This paper summarize a method of variance estimation considering multistage sampling design.The performances are compared with UCVE and the method considering multistage sampling design by simulating random sampling under different sampling schemes using real world data.Simulation showed that as primary sampling unit (PSU) sampling fraction increased,UCVE tended to generate increasingly biased estimation,whereas accurate estimates were obtained by using the method considering multistage sampling design.

ABSTRACT

The Study of Cardiovascular Risk in Adolescents (ERICA) aims to estimate the prevalence of cardiovascular risk factors and metabolic syndrome in adolescents (12-17 years) enrolled in public and private schools of the 273 municipalities with over 100,000 inhabitants in Brazil. The study population was stratified into 32 geographical strata (27 capitals and five sets with other municipalities in each macro-region of the country) and a sample of 1,251 schools was selected with probability proportional to size. In each school three combinations of shift (morning and afternoon) and grade were selected, and within each of these combinations, one class was selected. All eligible students in the selected classes were included in the study. The design sampling weights were calculated by the product of the reciprocals of the inclusion probabilities in each sampling stage, and were later calibrated considering the projections of the numbers of adolescents enrolled in schools located in the geographical strata by sex and age.


El Estudio de Riesgo Cardiovascular en Adolescentes (ERICA) tiene como objetivo estimar la prevalencia de factores de riesgo cardiovascular y síndrome metabólico en adolescentes (12-17 años) matriculados en las escuelas públicas y privadas de 273 municipios con más de 100 mil habitantes en Brasil. La población de estudio fue estratificada en 32 estratos geográficos (27 capitales y cinco conjuntos con otros municipios de cada macrorregión del país); además se seleccionó una muestra de 1.251 escuelas con probabilidad proporcional a su tamaño. En cada escuela se seleccionaran tres combinaciones de horario (matutino y vespertino) con año de la clase, y en cada combinación se seleccionó una clase. Todos los estudiantes elegibles en las clases seleccionadas fueron objeto de la investigación. Los pesos de diseño de la muestra se calcularon por el producto de los inversos de las probabilidades de selección en cada etapa de la muestra y después se calibraron teniendo en cuenta las proyecciones del número de adolescentes inscritos en las escuelas ubicadas en los estratos geográficos por sexo y edad.

Este artigo descreve a amostra da Pesquisa Nacional sobre Parto e Nascimento no Brasil. Os hospitais com 500 ou mais nascidos vivos em 2007 foram estratificados por macrorregião, capital de estado ou não, e tipo, e selecionados com probabilidade proporcional ao número de nascidos-vivos em 2007. Amostragem inversa foi usada para selecionar tantos dias de pesquisa (mínimo de 7) quantos fossem necessários para alcançar 90 entrevistas realizadas com puérperas no hospital. As puérperas foram amostradas com igual probabilidade entre as elegíveis que entraram no hospital no dia. Os pesos amostrais básicos são o inverso do produto das probabilidades de inclusão em cada estágio e foram calibrados para assegurar que estimativas dos totais de nascidos vivos dos estratos correspondessem aos totais de nascidos vivos obtidos no SINASC. Para os dois seguimentos telefônicos (6 e 12 meses depois), a probabilidade de resposta das puérperas foi modelada pelas variáveis disponíveis na pesquisa de base, a fim de corrigir, para a não resposta, os pesos amostrais em cada onda de seguimento.


Este artículo describe la muestra de la Encuesta Nacional sobre Partos y Nacimientos en Brasil. Los hospitales con 500 o más nacimientos en 2007 fueron estratificados por región, capital del estado o no, y tipo, y se seleccionan con probabilidad proporcional al número de nacidos vivos en 2007. Se utilizó un muestreo inverso para seleccionar los días de encuesta (mínimo 7), con el fin de lograr 90 entrevistas en el hospital. Se realizó el muestreo de las mujeres posparto, con igual probabilidad entre las mujeres elegibles que entraron en el hospital cada día. Los pesos iniciales son el inverso del producto de las probabilidades de inclusión en cada etapa y se calibraron para asegurar que las estimaciones del total de nacidos vivos correspondieran al total de nacidos vivos, obtenidos a partir de SINASC. Para los dos seguimientos telefónicos (6 y 12 meses después), la probabilidad de respuesta de las mujeres posparto fue modelada a partir de variables disponibles en la investigación básica, a fin de corregir, debido a la no-respuesta, los pesos de la muestra de cada ola de seguimiento.


This paper describes the sample design for the National Survey into Labor and Birth in Brazil. The hospitals with 500 or more live births in 2007 were stratified into: the five Brazilian regions; state capital or not; and type of governance. They were then selected with probability proportional to the number of live births in 2007. An inverse sampling method was used to select as many days (minimum of 7) as necessary to reach 90 interviews in the hospital. Postnatal women were sampled with equal probability from the set of eligible women, who had entered the hospital in the sampled days. Initial sample weights were computed as the reciprocals of the sample inclusion probabilities and were calibrated to ensure that total estimates of the number of live births from the survey matched the known figures obtained from the Brazilian System of Information on Live Births. For the two telephone follow-up waves (6 and 12 months later), the postnatal woman’s response probability was modelled using baseline covariate information in order to adjust the sample weights for nonresponse in each follow-up wave.
